| MEMBERSHIP (Temporary
hold on new members.) |

1.
Membership in FLIGHTS FOR LIFE, INC. shall
consist of Pilot members and non-pilot members. |
|
2.
Membership in FLIGHTS FOR LIFE, INC. shall be
open to all citizens of the United States, its Territories, and
interdependencies without discrimination, upon payment of the sums
provided in the by-laws. |
|
3.
Persons meeting the requirements of Section 3(a)
and (b) below shall be eligible for Pilot membership. |
(a) Those who possess a valid and current FAA pilot
certificate.
(b) Those who have a minimum of 100 hours PlC time. |
|
4.
Persons meeting the requirements of 3. (a) and
(b) above are eligible for pilot membership in FLIGHTS FOR LIFE, INC. |
|
5.
Both applications for membership and membership renewals will
be completed on a Membership Application/Renewal form approved by the
Board of Directors. This form will include Member/Pilot profile
information. It will also include a Waiver of Liability section. A
prospective member will complete both the application and the waiver and
submit the completed forms to the President at the next Flights for Life
monthly meeting.

6.
Each Flights for Life member will submit a new
Membership Application/Renewal form to Flights for Life at the beginning
of each calendar year and on it will update his or her profile
information. He or she will also complete the liability waiver section
with the renewal.
These should be submitted to the Flights for
Life Record Keeper by or no later than 31 Mar. |

7.
Once a new member completes the Orientation
Flight and first mission he should attend the next Flights for Life
monthly meeting to be recognized and receive his Flights for Life hat
and shirt. |

1.
Initiation fee for new members of FLIGHTS FOR
LIFE, INC. shall be set by the Board of Directors, shall be paid in
advance and shall commence on the first day of the month following
acceptance into membership. The initiation fee of $40.00 - subject to
change. |

2.
Upon payment of the initiation fee required for
membership in FLIGHTS FOR LIFE, INC., any person entitled to membership
shall become a member of FLIGHTS FOR LIFE, INC. and may continue such
membership by completing two missions (draw, platelet, or unscheduled
per calendar year and that a
current pilot profile is on file. |

1.
The Pilot-in-Command for all flights must meet
the following criteria: |
(a) Pilot Requirements for Day Cargo Flights
(b) 100 Hours minimum PlC time.
(c)
5 Hours minimum time in aircraft make and
model to be flown, or 10 Hours minimum if in high
performance aircraft.
(d)
25 Hours minimum time flying cross-country. |

2.
Pilot Requirements for Night Cargo Flights. |
|
Same
requirements as for day cargo flights, except as listed below: |
|
(a) 300 Hours minimum PlC time, or 200 hours
minimum with Instrument Rating. |
|
(b) 50 Hours minimum time flying
cross-country. |
|
(c) 30 Hours minimum night flight, or 15 hours with an Instrument Rating. |
|
(d) Recommend two pilots for night cargo
flights. |
